Payment/Insurance

Payment is expected at time of service. Fees are as notated here. Payment due is determined by insurance coverage (deductible/copay) at the time service is rendered.

We currently accept the following insurance:

Alliance Health
Anthem BC/BS
Aetna
Cigna
Humana
Tricare (Independently Credentialed)
Wellcare (Ky. Medicaid)

All other insurances will be considered SELF-PAY.
​ We are able to print Superbills for insurance reimbursement.


Fee/Attendance Agreement
Fees are due at each session unless we have a credit card on file which will be charged on Fridays following the session/service. PLEASE HAVE FORM OF PAYMENT READY PRIOR TO SESSION.
The cost of the session is based on your insurance provider/coverage.
Please contact your insurance prior to your initial appointment to determine coverage. The amount due based on insurance will be determined at the initial session. A Credit Card MUST be on file unless paid with check or cash to cover any cancellations/missed appointments. 

Please be mindful of the following:                 
Deductibles- The amount owed yearly prior to any insurance coverage. For example: If you have a $2000.00 deductible then you will be liable for medical services up to $2000.00 prior to any insurance pay out. If the deductible has not been met then you will be liable for the session.
Co-pay- The amount owed regardless of deductible being met until the OUT OF POCKET is met. The OUT OF POCKET is the amount of medical services you are liable to until insurance pays one hundred percent.
Co-insurance- Similar to the co-pay but based on percentage of what is billed rather than a fixed amount.         
 Some insurance policies have different deductibles for Mental Health services than for medical services.  
If any time you pay and then insurance covers more than expected: 
A. A credit will be applied to your account to cover future co-pays/deductibles. 
B. The amount will be refunded to you depending on form of payment (credit card, cash, check). 
Rates

• Individual Counseling $100.00 per hour 

• Marriage Counseling $100.00 per hour 

• School/Professional Consultation $100.00 per hour 

FEES NOT COVERED BY INSURANCE:You will billed through Square Invoicing. These must be paid prior to the next session. 

• Phone Consultation (Self, attorneys, school, etc.) $20.00 per 15 minute increments 

• Documentation/Requested Reports: $20.00 per 15 minutes increments.

• Records (Disability/Social Security/Court): $50.00 per report. 

• Emails not specifically related to sessions: $20.00 per 15 minute increments. 

Court Fees/Subpoenas When possible, please request a subpoena from your legal representative be sent to me for either a morning (8am-12pm) or afternoon (1pm to 5pm) to reduce waiting costs. I have to plan the scheduling of other clients around my court time. There are additional travel costs for court hearings outside of Vanderburgh or Warrick County. The party who is responsible for requesting that I be subpoenaed is responsible for the court costs unless otherwise identified in a current court order.  
  $750 retainer: For 1/2 day Vanderburgh or Warrick County Court hearing.  $1500 retainer: For a full day hearing in Vanderburgh or Warrick County. Other court related services: $150 per hour which will be determined at request and paid as a retainer.
RETAINER MUST BE PAID AT LEAST ONE WEEK PRIOR TO COURT HEARING.
** Court retainers are non-refundable if the case is cancelled without 48 hours notice. 
Attendance
In order to provide you with optimal care, your appointment time is reserved specifically for you. Our providers do not double book. We also provide complimentary appointment reminders to your family via telephone, text message, or email. In return, we ask that you carefully review our attendance policies below: 

Your Story Counseling & Services, LLC. retains credit card information during intake to cover cancellation/no show fees. These fees are processed same week as the cancellation/no show.

• Please arrive for your appointment on-time.  If you are more than 20 minutes late for an appointment, your therapist may not be able to see you for that session.  Please call or text us at (812) 661-9248 if you are going to be more than 20 minutes late.  

• Parents are expected to participate in sessions with children according to the following:
  •  Under the age of 5: At least every other session. 
  • Ages 6-12: At least one session per 4 . 
  • Ages 13-18: At least 1 session every 6.

• We ask that you provide us with a minimum of 24 hours notice if you are unable to make it to your appointment.   Insurance does not cover this fee. 

(NON-MEDICAID only) If you do not attend a session and also do not notify us of your absence (a.k.a., "No Call/No Show"), you will be billed for the missed session.  Insurance does not cover this fee.  

• No call/No Show Fee (Without Emergency): Cost of Session (Minimal of $100)
• Late cancellation fee (less than 24 hours without emergency): $50.00
  • ​Emergencies: Accidents, Sickness, Bereavement
  • Weather: If school is canceled in your district due to weather concerns then you will not be billed for a cancelled session. However please notify us as soon as possible.

Counselor cancellation: Sometimes your counselor will need to cancel due to above noted concerns (sickness, accident, weather, emergency sessions). If this is the case you will not be charged for the missed session for that day. You will be notified ASAP for any of these changes. 
Please inform us if you are not getting appointment reminders through email, voicemail, and/or text. These provide great assistance to ensure consistency and therefore prevent concerns with missed appointments.
It is likely if you are not receiving reminders then appointments are not scheduled. If you do not receive a reminder and your phone number/information has not changed and you suspect you are supposed to have an appointment please contact us as soon as possible to ensure you are scheduled.

• If you have numerous late cancellations or more than 2 "No Call/No Show" occurrences, you and/or your child will be unable to receive services for a period of 3 months.  We will provide you with phone numbers for other agencies and/or providers at this time.
